Position Details

🏡 Position: Registered Nurse Case Manager (Home Health)

📍 Location: Glendale, AZ Area

🕒 Schedule: Monday-Friday | 8:00 AM-5:00 PM

🎓 Paid Clinical Orientation: 2 Weeks

🚗 Mileage Reimbursement Included

💰 Competitive Pay-Per-Visit Rates

✅ Routine Visit: $60

✅ Start of Care (SOC): $112

✅ Recertification: $80

✅ Discharge Visit: $70

✅ Evaluation Visit: $55

✅ Non-Visit Activity (NVA): $40

❤️ What You'll Be Doing

✔ Conduct comprehensive patient assessments in the home setting

✔ Develop and manage individualized plans of care

✔ Administer medications and treatments as ordered

✔ Educate patients and families on disease management and care plans

✔ Coordinate care with physicians and interdisciplinary teams

✔ Monitor patient progress and adjust care plans as needed

✔ Support patients through recovery, rehabilitation, and improved quality of life

🎯 Qualifications

✔ Active RN License

✔ Minimum 1 year of Acute Care Nursing Experience

✔ Current BLS Certification (Hands-On CPR)

✔ Valid Driver's License & Auto Insurance

✔ Home Health Experience Preferred but Not Required

✔ Associate's or Bachelor's Degree in Nursing

Pay Details: $60.00 to $80.00 per hour
