Physical Therapist – Senior Care / PACE Program Schedule: Monday–Friday, 8:00 AM–4:30 PM Pay range: $90,000 - $110,000 Job Type: Full-Time Location: Colorado About the Opportunity We are seeking a compassionate and experienced Physical Therapist to provide evaluation and treatment services to adults age 55 and older. This role focuses on helping participants maintain or improve mobility, safety, independence, and quality of life through individualized physical therapy services. The Physical Therapist will work closely with participants, families, caregivers, medical providers, nursing and assisted living staff, and an interdisciplinary care team to develop and implement comprehensive Plans of Care. This is an excellent opportunity for a Physical Therapist who enjoys working with older adults and values collaborative, patient-centered care. Key Responsibilities Screen and evaluate participants for acute, restorative, or maintenance rehabilitation needs. Develop, implement, and update individualized physical therapy treatment plans in accordance with physician orders and the participant's Plan of Care.

Pay Details: $69.00 to $70.00 per hour
