📍 Territory:

• Glendale

• Central Phoenix

• Peoria

• Surprise

• Sun City

⏰ Schedule: Monday - Friday 8:00 AM - 5:00 PM No weekends required!

💵 Compensation: $10,000 Sign-On Bonus!

Visit Rates:

• SOC: $140-$150

• Recertification: $120-$130

• Evaluation: $120-$130

• Revisit: $90

Hourly Rate:

• $35-$40/hour (based on experience)

 

🌟 What You'll Do:

✅ Evaluate patients and develop individualized treatment plans

✅ Provide therapeutic exercises and interventions to improve mobility, strength, balance, and function

✅ Educate patients and caregivers on injury prevention, exercises, and self-care strategies

✅ Collaborate with nurses, physicians, and interdisciplinary healthcare teams

✅ Maintain timely and accurate patient documentation

✅ Help patients regain independence and improve their quality of life from the comfort of their homes

 

🎯 Qualifications:

✔ Active Physical Therapist License

✔ Master's or Doctorate Degree in Physical Therapy

✔ Minimum 1 year of Physical Therapist experience

✔ Home Health experience strongly preferred

✔ Current BLS/CPR Certification

✔ Valid Driver's License

✔ Reliable, insured vehicle

Pay Details: $35.00 to $40.00 per hour
