Occupational Therapist – PACE Program Pay Range: $40.49–$50.61 per hour, based on experience Sign-On Bonus: $5,000 Location: Maryland Employment Type: Full-Time Position Overview Join an innovative healthcare organization and help shape the future of community-based care. We are seeking a compassionate and experienced Occupational Therapist (OT) to provide comprehensive occupational therapy evaluation and treatment to participants in a PACE (Program of All-Inclusive Care for the Elderly) setting. The Occupational Therapist will provide acute, restorative, and maintenance therapy based on participant needs. Services may be delivered in a variety of environments, including the Day Center, nursing home, and participants' homes. The ideal candidate will be committed to promoting independence, safety, quality of life, and functional abilities for older adults and individuals with complex healthcare needs. Key Responsibilities Conduct comprehensive occupational therapy evaluations and assessments. Develop individualized treatment plans and care plans based on participant needs and goals. Provide occupational therapy services consistent with established goals and clinical recommendations. Deliver acute, restorative, and maintenance therapy as appropriate. Provide services in a variety of settings, including the Day Center, nursing facilities, and participants' homes. Evaluate participant progress and modify treatment plans as appropriate. Accurately document evaluations, treatment sessions, progress, and other required clinical information. Coordinate home modification programs designed to improve participant independence, accessibility, and safety. Train and educate family members, caregivers, and staff on techniques to maximize participant safety and functional abilities. Mentor, coach, counsel, and provide performance feedback to department staff as appropriate. Maintain knowledge of applicable laws, regulations, policies, and corporate compliance requirements. Participate in required training, educational programs, and staff meetings. Fulfill designated responsibilities during disaster recovery and business continuity situations. Collaborate with an interdisciplinary care team to promote positive participant outcomes. Minimum Qualifications Bachelor's degree in Occupational Therapy from an accredited program. Current, active, and unencumbered Occupational Therapist license in Maryland. At least 2 years of appropriate occupational therapy experience preferred. At least 1 year of experience working with a frail or elderly population preferred. Appropriate clinical placement experience may be considered in lieu of this requirement. Must meet required employee medical clearance/medical statement requirements. Strong communication, organizational, assessment, and clinical decision-making skills. Ability to work effectively with participants, families, caregivers, and interdisciplinary healthcare professionals.
